Advance Care Planning is the process of clarifying your values and your preferences for future health care, and identifying who you would like to speak for you if you are no longer able to make decisions for yourself. There are many Advance Care Planning resources for veterans. The Department of Veterans Affairs (VA) Geriatrics and Extended Care webpage has an entire section on Advance Care Planning with links to the VA Advance Directive form, and podcasts for you to listen to on advance care planning, choosing a Health Care Agent, how to be a Health Care Agent, and setting health care goals.
